of ADULT stem cells derived from cord blood,
peripheral blood, bone marrow and fat. The Group has
never been, and does not intend to become, involved
in the storage or expansion of embryonic stem cells. |

How can I get the collection
kit?

You should contact our office and request the
registration package via e-mail no
later then week 34 (late 7th month to
mid 8th month).

Please carefully fill the documents attached to
the registration package you received by e mail
and fax to our office at:

While sending the documents by fax please make
sure that your credit card information, your
signature and your package of choice are clearly
written (printed if possible).

Thereafter, please send the original documents by mail
to Cryo-Match, P.O Box 2004, Herzelia Pituach, Israel 46120.

Upon receiving your fax we will forward to you
by courier our Collection Kit.

On the Collection Kit you will find World
Courier 1-800 number and a job number which will
be required for identification when calling for
pick up .
Upon receiving the collection
kit what should I do?

Upon receiving the collection kit please
carefully read all attached documents in order
to allow us to answer any question you may have
prior to the delivery.

Send us immediately the required blood test
results by

The collection kit you receive is close. Please
do not remove the interior sticker that is to be
removed in the delivery room by the doctor /
midwife ONLY.

Please fill all personal information required in
documents attached to the kit.

Please make sure that until you get to the
delivery room, the kit is kept in room
temperature and avoid keeping it in your car.
Please make sure to take the kit and all documents
and courier's phone number with you
to the hospital.
